Profile

Dr Jean Lee is a Senior Consultant Emergency Physician at Sengkang General Hospital (SKH). She is passionate in initiatives focused on empowering the elderly with their health management and overall wellbeing.

As Director of the new Department of Transitional Care and Community Medicine at Sengkang General Hospital, she is responsible for overseeing the development and progression of Transitional care and Community Medicine programs to right site health services and focus on upstream preventive measures to bring about optimal social and physical health management of patients in the community. 

This includes aligning population health interventions in the Northeast with Singhealth cluster directions and optimizing healthcare capacity sharing.

As Clinical Lead Community Partnerships, her role includes optimizing patient journeys for effective and cost-efficient care, enabling community partners (eg nursing homes, Intermediate Long Term Care facilities) with care pathways and decision support to assist their staff to manage their residents at the top of their license and minimize unplanned or inappropriate hospital admissions. She believes firmly in addressing the social determinants of health by connecting social care services to the patient care journey and forming the requisite partnerships.

As Director of Geriatric Emergency Medicine (GEM), she oversaw the setup of this relatively new discipline in SKH's Emergency Department during the development of the SKH, where elderlies coming through the emergency department are screened for frailty and multi-disciplinary interventions implemented to optimize their wellbeing where necessary. 

She is currently Co-chair of Singhealth's Academic Clinical Program Geriatric Emergency Medicine(GEM) as well as co-lead of the National Chapter for GEM Clinical Services.

Prior to joining SKH, she was Director of Clinical Operations and Manpower Singapore General Hospital's (SGH) Emergency Department where she worked from 1998 until 2016. At SGH, she was instrumental in the transformation of the clinical workflows of the Emergency Department and overseeing the restructuring of the department to deal with the ever increasing patient numbers.

She has won and led Agency of Integrated Care and Singhealth's Population health Development Funded Nursing Home projects and has done stellar work with the Nursing Homes(NH) in the northeastern region of Singapore. She was awarded the Healthcare Manpower Development Programme award to study Geriatric Emergency Medicine at McGill University Montreal and Sunnybrooke Health Sciences Centre Toronto.
