Please provide us with these documents:
a) Completed Consent for Release of Medical Information Form Download it here: Consent Form - Release of Medical Information.pdf
b) Copy of patient’s identification documents e.g. Copy of patient’s NRIC (front and back)
c) Any relevant supporting documents and forms
NOTE: Request for medical report will only be processed upon receipt of your documents, consent form (Consent for Release of Medical Information) or insurance form, and appropriate payment.
A patient who is a minor may apply for a medical report with the consent of his/her parent or legal guardian
If the patient is deceased or mentally incompetent, consent is required from the appointed representative of the estate. If there is no representative appointed for the estate, a separate Letter of Undertaking has to be completed by all immediate next-of-kin of the patient. Copies of the death certificate and other supporting documents e.g. marriage certificate, birth certificate, and letters of administration, as proof of your relationship to the patient must be submitted together with the application.

As a general guide, please allow 4 - 6 weeks for processing of requests. However, we may experience a delay in processing some requests due to:
The regular consultation and hospitalisation fees paid do not include the cost for medical report. A fee to be levied for this additional service of providing medical reports to cover the hospital's administrative costs and the doctor's professional inputs. This is a standard practice across all Public Healthcare Institutions.
